Appointments can be made by calling HVIC on 02 60565285. When you call HVIC, one of our reception staff will make an appointment with the necessary person, be it a Pension Officer, Welfare Officer or an Advocate.

In urgent situations where a Welfare Office is required after hours, the Senior Welfare Officer (Errol Smith) is available on: 0419632662.

What is needed for an appointment?

In order to complete your claim for compensation, you will need to bring along some important information to your interview. This will save time in processing and also give you the best possible chance of obtaining compensation for your injury, illness or condition that you maybe entitled to:

Proof of identity (POI) is required if you have not made a claim for compensation through the Department of Veterans’ Affairs. POI is also required when making a claim for Service Pension;

DVA number if issued or treatment card;

Any military medical documentation associated with your injury/condition;

Any medical documentation associated with your injury/condition since discharge, including any associated specialist reports. ‘Specialist reports are very important to your case’;

Enlistment and discharge Medical Board health examination;

Record of Service which should include; date of enlistment, date of discharge and any operational service, including dates of operation/s;

Accident incident report AC563;

Any previous correspondence or decisions you have had with the Department of Veterans’ Affairs;

YOUR Bank details and Tax File Number; and

Lastly, you might also wish to bring your partner/wife to the interview. In many cases your partner will be able to assist with describing how your disability affects you and your family.
